In September 2004, a 39-year-old Florida man fell victim to the mummy's curse and fell approximately four feet off the loading platform as he was attempting to step into the ride vehicle. In 2011, a rider was struck by an object while riding the Dragon Challenge, injuring his face and leg. As a result of this and a previous incident in which a rider was blinded, Universal officials announced that the two roller coasters would no longer operate simultaneously, pending an investigation into both incidents. In October 2011, officials decided to shelve the dueling aspect of the ride indefinitely.

In August 2002, a woman was pulled from the audience to participate in the show. After being frightened by one of the on-stage monsters, the woman fell off the stage in fear, landing on the steps of the theater. She was rushed to the hospital and was released shortly afterwards without any further injuries.

Prior to last call in 2014, a man was shot and killed by police officers at Universal CityWalk after they were called in to take care of a fight in a crowded club. They were confronted by the man carrying a gun who had fired into the crowd. The officers shot and killed the man.

On June 13, 2008, a mechanical failure led a Simpson ride vehicle and its occupants to be sprayed with vegetable oil. Universal Studios gave these guests a change of clothes and the opportunity to shower on site. No injuries were reported and there was no word on why there was so much unused vegetable sitting around.

On the opening weekend of Universal Studios in 1915, studio chief Carl Laemmle orchestrated a flying spectacle to dazzle the thousands of guests who had traveled across country to see the unveiling of "the greatest motion picture city in the world."
A pilot named Frank Stites was performing stunts mid-air when he fell from his plane and died on impact. Rumor has it that his ghost haunts the Universal City backlot.

Littlefield, 34, was arguing with a Universal Studios security supervisor in a parking deck about whether or not he could drink alcohol on the premises when he flashed a fake badge. When he was being arrested, officers noted that he seemed to be under the influence of alcohol.
