  This trope is actually one of the main themes of Fullmetal Alchemist. When Edward and Alphonse Elric lose their mother to an unknown disease, they decide that they aren't simply going to accept their loss. Instead, they'll bring her back to life through alchemy.

  The consequences of doing so are severe. Not only do they fail to resurrect their mother, they lose Edwards arm and leg, as well as Al's entire body in the process. They spend a long time mired in depression and regret but eventually resolve to move forward and figure out how to get their bodies back.

  (#2) Tenya Iida Became A Vigilante In 'My Hero Academia'

  After Tenya Iida's older brother Tensei is assaulted by Stain, the normally rule-abiding hero-in-training is so furious that he decides to get revenge. This means breaking the law, since without a hero license he isn't allowed to go after criminals on his own. 

  He manages to track Stain down after taking an internship in the same city where he was last seen. The regret kicks in when he realizes how profoundly outmatched he is, and begins to think that he won't make it out of the confrontation alive.

  In the end, Midoriya and Todoroki help him out, and they successfully defeat Stain. Iida still regrets his actions despite the positive outcome, because he was reckless and strayed from his core values.

  When you're the arrogant prince of Saiyans, sometimes you make decisions that aren't in your best interest because of your pride. That's what happens to Vegeta when he allows Cell to absorb Android 18 and achieve his final form, Perfect Cell. He could have stopped him from doing so, but he chose to allow Cell to grow more powerful after Cell taunts him for not challenging himself with powerful opponents. 

  Vegeta, Future Trunks, and Krillin are all taken out by Cell - a situation that could have been avoided if Vegeta just had a handle on his pride. Sure, Cell is eventually defeated and everyone recovers or is revived, but there was no reason to go through all that pain.

  One of the worst things that happens in the entirety of Code Geass happens because of an honest mistake. When Lelouch is explaining to Euphemia how his Geass worked, he accidentally activated it. Intending to give an example about the extreme things he could force another person to do, he accidentally ordered Euphemia to exterminate the Japanese race. With no willpower of her own to do otherwise, she started shooting at a group of people she had previously been fighting to protect and perished herself in the ensuing chaos. 

  While this wasn't exactly a conscious decision on Lelouch's part, he still chose to say the words aloud while knowing that his Geass was unstable. He couldn't possibly have regretted it more.

  (#5) The Pilaf Gang Gets Nothing From King Piccolo After Reviving Him In 'Dragon Ball'

  The members of the Pilaf Gang are some of the least threatening villains in the Dragon Ball universe, which may be why their plan to rise to power blows up in their face. Emperor Pilaf and his cronies Shu and Mai help resurrect King Piccolo under the assumption that he will give them a portion of the world after he takes it over. Instead, King Piccolo gives them absolutely nothing, making them instantly regret going through all the effort to find the rice cooker he was trapped in at the bottom of the sea.

  (#6) Senku Revived A Dangerous Person In 'Dr. Stone'

  While being chased by wild animals, Senku decides to use revival fluid to free Tsukasa Shishio from his stone form. At first, this seems like a good idea, because Tsukasa is extremely powerful and can fight off wild animals and hunt for them. But very quickly, it becomes clear that reviving Tsukasa might have been a terrible idea. That's because Tsukasa wants to selectively revive humanity and destroy everyone he thinks is unworthy of being a part of his new world - and his criteria is based solely on age. Senku thinks this is murder and is intent on using science to revive everyone - which makes the two of them enemies and puts Senku's life in danger. 

  The anime has yet to reveal whether Senku will ultimately come to regret his involvement with Tsukasa, but in the meantime, it's causing him some serious trouble.

  (#7) Tanjiro Kamado Wasn't There When His Family Needed Him In 'Demon Slayer: Kimetsu no Yaiba'

  It would be cruel to blame Tanjiro Kamado for what happened to his family while he wasn't there to protect them, but that doesn't stop him from feeling a sharp sense of guilt and regret. While he was out running errands, his entire family was taken out by a marauding demon - except for his sister Nezuko, who was transformed into a demon. 

  Tanjiro decides to deal with his sense of pain and remorse by not just seeking out a way to turn Nezuko human again but also slaying demons while offering them as many opportunities for reform as possible.

  Erina Nakiri idolizes Jōichirō Saiba, believing him to be the greatest chef in the history of the world, and hoping to one day achieve his level. When she discovers that Yukihira Soma, the classmate she constantly mocked and looked down on, is actually her idol's son, she's utterly horrified as she reflects on how poorly she treated him.

  Any member of the Sohma family who makes the family head angry is going to feel some form of regret afterward. Young Hiro Sohma learns this the hard way when he tells Akito that he's in love with Kisa Sohma, another member of their enormous clan.

  Akito refuses to tolerate romantic feelings of any kind from the cursed members of the clan, and retaliates by beating Kisa severely. Hiro hadn't known that Akito would react this way, but after it happened he's filled with anger, shame, and regret. He starts avoiding Kisa and becoming combative and sarcastic when they do talk, because he can't stand that he was unable to protect her.

  In order to prevent her friend Sayaka from entering a dangerous battle, Madoka tosses her soul gem off of a bridge into oncoming traffic. She thinks that this will stop her from being able to transform into a magical girl, but she's mistaken. Actually, the soul gem is, quite literally, a magical girl's soul. That means that if it's separated from her body, her body loses consciousness and potentially its life. If the soul gem is destroyed, it's game over.

  Though Homura is able to retrieve the gem unharmed, Madoka is horrified by the fact that she unknowingly put her friend in danger - and she's even more horrified to learn that magical girls no longer truly inhabit their bodies.

  Sakura gets a lot of flak from the Naruto fandom. And it's moments like these that add fuel to the fire. Even after Obito tells her very clearly that she needs to destroy his Rinnegan before Madara appears, Sakura hesitates. Before she can make a decision, Madara shows up and steals the Rinnegan for himself, creating even more challenges and conflicts for the protagonists. The war would have ended a lot more quickly had she not hesitated.

  Yamato doesn't see this as a big deal, since he sees his relationship with Megumi as wholly platonic and has no intention of cheating on Mei. But Mei is only hearing secondhand rumors and is deeply upset. Yamato doesn't get why until their mutual friend Aiko asks how he'd feel if the situation were reversed. He's immediately flooded with regret over his callousness and resolves to talk things over properly with Mei.

  Poor Gentle. He really did have good intentions. But unfortunately, his "heroic" act ended up doing more harm than good. In his youth, Gentle Criminal strived to be a Pro Hero even though many told him he wasn't cut out for that career path. One day, he sees a man about to fall from a high building and takes this chance to prove that he's a hero. He uses his quirk to create an elastic surface that the man can fall safely onto. But it ends up hindering a Pro Hero's attempt to rescue the man. As a result, the victim falls to the ground and is critically injured. Gentle is charged by the police for obstructing justice and quickly disowned by his family, leading to his transformation into a villain.

  (#15) Mari Terahara Poisoned Her Bandmate In 'Case Closed'

  Mari Terahara has a troubled history with Tatsuya Kimura, the lead singer of the band she managed, Lex. Before Tatsuya formed Lex, he and Mari were in a band together, which is why Tatsuya invited her to manage his new band. Harboring intense romantic feelings toward him, she accepted the offer and got plastic surgery to keep him interested. Tatsuya didn't react kindly to her decision, opting to mock her instead of being supportive. Furious, she decides to smear cyanide on his jacket, which leads to his ultimate death once he gets cyanide on his food after a show. 

  Why does Mari regret her actions? Because it turns out that Tatsuya actually loved her, and reacted negatively to her plastic surgery because he thought she was perfect before. One would hope that you'd regret poisoning one of your band members regardless of whether or not you were mistaken about his feelings for you, but oh well.

  (#16) Hiyori Moritani Put A Hit Out On Manabe In 'Kotoura-san'

  Furious that Manabe seems to be falling in love with Kotoura instead of her, Moritani sends the employees at her family's dojo to beat him up. She accomplishes this by telling the employees that Manabe has been stalking her, which leads them to believe that he's far more dangerous than he actually is. Moritani only meant for him to be roughed up a little, but the employees beat him so badly that he needs to be hospitalized. Moritani is horrified by her own behavior, a feeling which triggers her to stop being a bully in general.

  (#17) Kazuma Tried To Save A Girl In 'KonoSuba'

  Sometimes, it's okay to not be the hero. And Kazuma had to learn that the hard way. In the beginning of the series, Kazuma tries to save a girl from an oncoming truck, and subsequently dies in the process. But when he's reincarnated in a fantasy world, he learns the humiliating truth. His noble sacrifice was done in vain. The "truck" that he saved the girl from was actually a slow-moving tractor. And the girl was never in danger in the first place as the tractor was planning to stop. Simply put, Kazuma died from shock and fear of being hit by an imaginary truck. He even wetted his pants. Quite the regrettable death.

  (#18) Takao Kasuga Stole A Gym Uniform In 'Flowers Of Evil'

  One bad decision pretty much completely ruined Takao Kasuga's life. He's in love with his classmate Nanako Saeki, and he acts on that love by stealing her gym uniform. He puts a lot of stock in being an idealized person, so he immediately starts castigating himself for his behavior and trying to figure out how to safely return the uniform without getting caught.

  Unfortunately for Takao, Sawa Nakamura found out about his thieving ways, and she plans to manipulate him until unleashing the devian she believes sleeps inside of him.
